form(s) per applicant properly completed and signed. Signature

Two recent 2 x 2 passport type photograph(s), in color,
front view and with a plain/light background. |

Proof of Departure:
Computer generated flight itinerary from the airline or
a travel agency, copy of airline ticket or a corporate letter
stating travel plans. A signed letter on the travel agency’s
letterhead confirming round-trip booking may also be submitted. |

Business Letter: Letter from employer
or sponsoring company, on company letterhead, introducing
the applicant, indicating the applicant’s employment status/position
held in the company, and clearly stating the purpose of
visit to Tanzania. The business letter must also indicate
who will be financially responsible for the applicant and
must provide detailed contact information in host country. |

Medical Requirements:
International certificate of vaccination for Cholera and
anti-Malaria medication. |

Financial Requirements:
Evidence of sufficient funds to support stay in destination
such as pay stubs, traveler’s cheques, and/or most recent
bank statement. |

Status: Copy of U.S. Permanent Resident Card or
valid visa. US visa holders should have original I-94 or
I-20 stapled to their passport. |

Special Instructions:
Effective Monday, September 10, 2007, the Embassy of Tanzania
will not allow pick up of completed visas/documents on Fridays.
Pick up of completed visas/documents will occur from Monday
through Thursday. Submission of visas/documents will occur
from Monday through Friday. |

Visas are valid for three months, single entry from
the date of issue and allow 28 days stay. |

Processing Time:
Visas are usually processed in 12 working days. Rush
and Emergency service may be available for additional fees.

to retrieve the visa in less time than the amount required
