HOW TO FILL OUT THE NEW RUSSIAN VISA FORM
Read this information carefully, and then fill out your online
Russia visa application.
RUSSIAN ONLINE APPLICATION FORM
The Russian government has introduced a new online application form.
Effective July 1st, 2011, no other visa application forms will be accepted
by theRussian Embassy or Consulates in the US.
Because this form is created and hosted by the Russian government, Travisa
cannot provide technical support such as accessing information, reprinting
application forms or correcting errors on completed visa applications.
Therefore, we have created this guide to assist you with the online
application process.
You can fill out an online Russia visa application for yourself or for
another person. Once completed, the Russian Visa Application Form must be
printed, signed by the applicant, and submitted for visa processing along
with all other
Russia Visa Application Requirements.
BEFORE YOU START THE RUSSIAN VISA APPLICATION FORM
Note: The Russia Visa Application online form requires
information about inviting organization, as well as the date
of entry and departure (dates of validity for the visa). This information
can be found on the Russia Visa Invitation. (
Russia Visa Support).

questions on your Russia Visa Application Form. Note: you will need to
request your Russian visa support before you fill out the online visa
application form.
INFORMATION YOU WILL NEED TO COMPLETE THE RUSSIAN VISA APPLICATION FORM
Important: Once printed, no
changes or corrections can be made except by accessing the Russia visa
application form online. Handwritten corrections are not accepted by the
Russian Embassy or Consulate.
- What type of visa is needed: (business, tourist, homestay?)
- Personal information: Name, (any other name ever used), Gender, Marital status, Date of birth, Place of birth, Profession
- Citizenships: Present citizenship, other
citizenships ever held
- Have you formerly had USSR or Russian citizenship (if yes): Date and
reason for loss of Russian citizenship
- Permanent Address: Street number, City, State, Zip code, Country, Home phone, Fax, Email
- Place of work or study (if working/studying): Name, Position,
Employer/School Address, City, State, Zip code, Phone, Fax, Email
- Passport information: Type, Number, Date of issue, Date
of expiration (note: the type of passport for most applicants will be
“Tourist”. A Tourist passport means an ordinary travel passport.
- Visa information: Type, Number of entries, Itinerary in Russia (cities your plan to visit)
*Note: this information must match the official
invitation from Russia exactly.
- Date of departure from USA, Date of entry in Russia, Date of departure from Russia
(note: the date of entry and departure for Russia must cover
the entire period of Travel to Russia and must match the Russian visa
invitation; the visa validity will be based on these dates)
- Russian institution or organization to be visited: - For
Tourists: Name and Reference Number of the Host Tourist Company issuing the
voucher and confirmation (generally found on the seal), - For Businessmen: Name
and address of the Host Organization, -For Private Persons: Names and Home
Address of the Host
*Note: this information must match the official
invitation from Russia exactly. If you are obtaining your visa support
invitation from Travisa Visa Service, we will provide you with this
information.
- Person or hotel in Russia who you plan to stay with (name and
address) - For Tourists: the hotel and city listed on the application
must also be listed on the Visa Invitation (voucher and confirmation)
- Are you going to be part of a tour group in Russia? (if yes): Indicate the name and reference number of the tour group
- Do you have a medical insurance policy valid in Russia? (if yes): Provide detail
- Who will finance your trip to and stay in Russia?
- Parents information: Father/Mother's name (First, Last and Middle name – the Russian government calls the middle name the “Patronymic Name”.
- List all countries you have visited in the last ten years and indicate the year of visit
- List all countries that have ever issued a passport to you:
(if you have only ever had a US passport, list USA).
*Note: You must not leave this question blank. Any
country that has issued a passport to you in the past must be listed,
including the country of your current citizenship.
- Previous jobs held excluding the current one: Provide employer Name, Address, Position, Phone, Supervisor, Start/End Date
- Previous education institutions attended excluding high school and current institution: Provide institution Name, Course of study, Address, Phone, Start/End Date
- List all professional, civil or charity organizations with which you are involved
if none, answer “None”
- Do you have any specialized fire-arms training? if
Yes explain details, if none, answer “None”
- Have you ever lost your passport or had one stolen?
- Have you ever performed military service? if yes, indicate the country, rank, occupation, branch/date of service
- Have you ever been involved in an armed conflict? (either
military service or as a victim) If yes, please specify?
- Have you ever been arrested or convicted for any offense? (if yes): Provide detail
- Have you ever been afflicted with a communicable disease? (if yes): Provide detail
- Have you ever been a drug abuser or an addict? (if
yes): Provide detail
- Have you ever been refused a Russian visa? (if
yes): Provide detail
- Have you ever been issued a Russian Visa? (if yes): When were you issued your last Russian visa?
- Has your Russian Visa ever been cancelled?
- Have you ever tried to obtain or assisted others to obtain a Russian visa by providing misleading/false information?
- Have you ever overstayed your Russian visa or stayed unlawfully in Russia?
- Have you ever been deported from Russia?
- Do you have any relatives staying in Russia now? (If yes, full name,
relationship, date of birth, permanent address)
- Do you have any specialized skills, training or experience related to fire-arms?, explosives/nuclear matters?, biological/chemical substance? If yes, please specify
STARTING THE ONLINE RUSSIAN VISA APPLICATION FORM
- When you access the online Russia application, you will need to select “REGISTRATION”
- Under “Request an Account” enter your name, and enter the numbers you see, then click “Create User”
- Write down the Declarant ID and Password, and select “I remember my Declarant ID and Password”, then click “Finish”.
Important: if you lose this information, you will not be able to access, edit or reprint your application. You will need to re-register to fill out a new application.
- On the “About our Web-Site” page, review the information, then click “APPLICATION”
- Review the Terms and Conditions, and click “Yes” that you agree, in order to continue to the Russian online application form.· To edit a saved application, look for Saved/Completed application under “LIST”
- *Note: when you
complete the application, you will be asked to set an appointment for your
application. The date can be flexible, but you must choose the city where
you intend to send the application. Enter the same city as the Travisa
location where you will submit your request.
PRINTING THE COMPLETED RUSSIAN VISA APPLICATION FORM
Important: Improperly printed Russia visa application
forms may delay the processing of your visa.
- Important: Russian Consulates in the US (except New York) require the
application printed double sided (back-to-back); they will not accept
applications on two pages.
- You will need Adobe®Reader to open the file. (download ADOBE® Reader)
- Open the file in ADOBE® Reader
- Use File menu, then Print
- In the print menu, select the following options
- Page Scaling, choose None
- Do not check Auto-Rotate and Center
- Do not check “Chose Paper Source by PDF page size
If you are not able to print the Russia Visa Application Form from a
web-browser, then save it to a file and print the file.
