WHAT YOU NEED

Please print and complete two (2) copies of the Letter of Authorization.

Application Form for a UK passport for children under the age of 16 (C2)

Application Form for a British National (Overseas) passport (BNO-A) (HONG KONG PIC HOLDERS ONLY)

Application Form for a British National (Overseas) passport for a child under 16 (BNO-B) (HONG KONG PIC HOLDERS ONLY)

Please note: If you have lost your passport and are applying for a new one you must also complete and sign Form LS01

Flight Itinerary is required

Two Passport Photographs.

Proof of UK citizenship. Your expired/expiring UK passport OR An official UK birth certificate.

Letter of Consent - Young adults aged between 16 and 18 must have written parental consent for the issue of a new passport.

Complete the Global Reservation Form. This is Travisa's Service Order Form. You must include a copy of this form with all other supporting documents.

Same day/Next day - $149.00

Print this page and proceed to the Global Reservation Form. Complete all the fields with the necessary information and review the screen for accuracy. PRINT THE COMPLETED FORM and proceed to the Letter of Authorization. PRINT 2 COPIES of the form, fill them out and sign them.
