BRITISH PASSPORT RENEWAL REQUIREMENTS |

The following are requirements for a BRITISH
passport RENEWAL and for a new PASSPORT BOOK in case the visa pages
of a valid passport have been filled: |

| FedEx the following documents to Travisa for processing:

Original current or expired
British Passport. |
| 2 |
One passport application form per applicant completed, printed
and signed by both the applicant and a countersignatory. Signatures
must be original on each application. Improperly completed forms may
delay the processing of your application.

Two recent passport type photographs in
color, front view, full face, and with a light background.
Note: one photo must be endorsed on the back by a countersignatory.
The photo must be endorsed with a statement “I certify that this is
a true likeness of [applicant name]”. The person who endorses this
photo must also sign the application.

Print and sign three
Letters of Authorization. This
form with your signature authorizes Travisa to submit your passport
request. Three copies are needed with original signatures |

Letter of
Consent - Young adults aged between 16 and 18 must have written
parental consent for the issue of a new passport.

| 6 |
Computer generated flight
itinerary from the airline or a travel agency, copy of airline
ticket or a corporate letter stating travel plans (if applicable).
An itinerary is required for requests to process in less than 2 to 3
weeks. |
| 7 |
Complete the Global Reservation Form. This is Travisa's Service Order Form.
You must include a copy of this form with all other supporting documents.
