Current valid, signed 10 year U.S. passport and expiring 2nd valid passport. |

Two
identical, passport size, color photos, 2x2 with white background. Click

3 |
Completed DSP-82 Form.
This form must be completed online and printed on two pages (not back to back).
The passport office will not accept hand-written DSP-82 applications. This form
must have the original signature of the applicant.

Two Letters of Authorization. Please select the authorization letter that
matches the processing location and print two copies. Both must have original
signatures |

5 |
Proof of departure date- computer generated flight itinerary from the airline
or a travel agency, copy of airline ticket or a corporate letter stating US
departure by private aircraft.

6 |
A letter, signed by the applicant, addressed to the Passport Office, explaining
why a second passport is necessary. |

|
A request form for 2nd valid passport is required by certain passport agencies.

Complete the Global Reservation Form.
This is Travisa's Service Order Form. Once you have completed this form, you may
call Travisa to make a reservation for your passport processing. This form must
be printed and included with application materials sent to Travisa.
You must fill out the Global Reservation and call to make a reservation in
advance for emergency same/next day passport processing. The service fee is non
refundable.
