NAME CHANGE REQUIREMENTS |

PASSPORT IF: |
- You are able to submit your most recent US passport.
- you can document that your name has legally changed since the passport
was issued.
- yOUR valid PASSPORT IS NOT HEAVILY DAMAGED OR MUTILATED.
- Your most recent passport was
issued when you were 16 years of age or older.

Valid/signed U.S. passport. |
|
2 |
Two
identical, passport size, color photos, 2x2 with white background. Click here
for detailed specifications. |

If your current passport was issued within the past year: One completed
Reapplication form DS5504
form. This form must be completed online. The passport office will not accept
hand-written DS-5504 applications. This form must have the original signature of
the applicant.
If your valid passport was issued more than one year ago:
One completed DSP-82 Form.
This form must be completed online and printed on two pages (not back to back).
The passport office will not accept hand-written DSP-82 applications. This form
must have the original signature of the applicant. |

Two Letters of Authorization. Please select the authorization letter that
matches the processing location and print two copies. Both must have original
signatures. |

Proof of departure date - computer generated flight itinerary from the airline
or a travel agency, copy of airline ticket or a corporate letter stating US
departure by private aircraft. This is not required in Chicago for 3-10 day
processing. |

Court-Certified Name Change Decree OR Marriage Certificate (Not a Marriage
License) OR Divorce Decree, in the form of a certified copy. |

7 |
Complete the Global Reservation Form.
This is Travisa's Service Order Form. Once you have completed this form, you may
call Travisa to make a reservation for your passport processing. This form must
be printed and included with application materials sent to Travisa.
You must fill out the Global Reservation and call to make a reservation in
advance for emergency same/next day passport processing. The service fee is non
refundable.
