REQUIREMENTS FOR NAME CHANGE FOR US PASSPORT

Send the following documents to Travisa for processing.
1 Valid/signed U.S. passport.
2 2 (two) Passport Size color photos (2x2 with white background). Click here for details.
3 One completed Reapplication form DS5504 (within one year of passport issuance for name-change). This form must be completed online. The passport office will not accept hand-written DS-5504 applications. One completed Renewal Form DS82 (if the passport was issued more than a year ago and less than 15 years). This form must be completed online. The passport office will not accept hand-written DS-82 applications.  If your passport  was issued more than 15 years ago, you need to apply for a new passport.

Court-Certified Name Change Decree OR Marriage Certificate (Not a Marriage License) OR Divorce Decree, in the form of a certified copy.

6 Signed Letter of Authorization. 
7 Proof of departure date- computer generated flight itinerary from the airline  or a travel agency, copy of airline ticket or a corporate letter stating US departure by private aircraft.

Attention You must call to reserve a space and fax a copy of the global reservation form to confirm your reservation (same day/next day/48 hours and 3 to 10 days processing).
The service fee is non refundable.

Complete the Global Reservation Form. This is Travisa's Service Order Form. You must include a copy of this form with all other supporting documents.

Travisa's Service Fees are - 3 to 10 working days processing - $99.00
- 24 and 48 hours processing - $179.00
- Same day processing - $199.00
