RENEWAL PASSPORT REQUIREMENTS |

| Passport is renewable if: | ||||||||||||||||||||||||
| 1. You are able to submit your most recent US passport. 2. Your most recent passport was issued in your current name. 3. Your most recent passport was issued within the past 15 years. 4. Your most recent passport was issued when you were 16 years of age or older. If you answer no to any of these, First-time instructions apply. |

2 Signed Letters of Authorization. | |||||||||||||||||||||||

2 Passport photos (2x2 with white background). | |||||||||||||||||||||||

One completed application form DSP-82.This form must be completed online. The passport office will not accept hand-written DS-82 applications. | |||||||||||||||||||||||

Check for $135.00 payable to U.S Department of State. | |||||||||||||||||||||||

Proof of departure date- computer generated flight itinerary from the airline or a travel agency, copy of airline ticket or a corporate letter stating US departure by private aircraft. | |||||||||||||||||||||||

Passports are processed according to U.S. Departure Date.
You must call to reserve a
space and fax a copy of the global reservation form to confirm your reservation (same
day/next day/48 hours and 3 to 10 days processing). The service fee is non refundable. |

Complete the Global Reservation Form.
This is Travisa's Service Order Form. You
must include a copy of this form with all other supporting documents. Travisa's Service Fees are - 3 to 10 working days processing - $99.00  - 24 and 48 hours
processing - $179.00- Same
